Information of the Special fixed benefit payment

How to apply:

Please send the following documents in the enclosed “reply envelope”. At a later date, it will be transferred to the specified account.

Required documents:

  • The completed application form.
  • A copy of the householder’s bankbook that you want to receive the funds. (Front cover and inside front cover)
    ※ A copy of the cash card is also acceptable.
  • A copy of the householder’s identification card such as a Resident Card (Zairyu Card).
    ※ Please contact us if the applicant is not the householder.

Application Period for both online and mail:
 From May 20, 2020 to August 19, 2020
※ The postmark should be before/on Aug. 19, 2020.
 We will NOT accept the application form after Aug. 19, 2020
